Accredited Care Services is a community based provider supporting people with intellectual and developmental disabilities lead healthier lives. We support their daily operations and promote their full inclusion and participation in the community.

Supported Living

Supported Living arrangements are apartments / single family homes for up to three (3) related or unrelated persons with developmental / intellectual disabilities. Each person is supported through the 1915 DDA Medicaid Home and Community Based Waiver (HCBS). There are a multitude of wrap-around services that are offered through the HCBS and are authorized by DDS. Such services include, but are not limited to, Nutrition, Family Counseling, Occupational Therapy, Physical Therapy, and Speech Therapy. This is a good option for people who seeking to live more independently and with a limited number of housemates

You can expect from this program:

  • Direct Support Professionals help you live independently when you move into a home that you lease or own
  • Dedicated Direct Support Professionals up to 24/7 with and without overnight supports
  • Support to make your own choices
  • Assistance, if required, with activities for daily living (ADLs) which could include bathing, dressing, eating and mobility
  • Assistance with Instrumental Activities of Daily Living (IADLs) which could include grocery shopping, nutritional planning, household chores, money management, decision making and meal preparation

HOST HOME

The Host Home Program allows people with developmental disabilities to live with a caregiver, family of their choice, other than a parent, spouse, or guardian, within the community. The Host Home service is provided through a Host Home Provider (such as Accredited Care Services) within the Department of Disabilities Administration (DDA). Host Home living arrangements are a great option for people looking for a more family-oriented model. Becoming a Host Home is a great way to offer a home and support structure to a person wanting this type of living arrangement. Host Homes operate like independent contractors with earnings receiving tax-free status.

IN-HOME SUPPORTS

: In-Home Supports are supports that are provided by a Direct Support Professional to a person that lives in their natural home for up to 8 hours per day 7 days per week. The purpose of this service is to keep people at home with their family by providing the necessary level of care in the natural home; allowing the family to work outside the home and do other activities that they would not be able to do without assistance. The service can be utilized by family to assist with taking people to medical appointments, community events and activities or to assist the person receiving services to enhance their daily living skills.

You can expect from this program:

  • Direct Support Professionals to come to your home and work with you on desired skills
  • Drop-in supports from 2 to 10 hours per week based on your scheduling needs
  • In-person or a combination of in-person and virtual supports
  • Support to make your own choices
  • Assistance, if required, with activities for daily living (ADL’s) which could include bathing, dressing, eating and mobility
  • Assistance with Instrumental Activities of Daily Living (IADL’s) which could include grocery shopping, nutritional planning, household chores, money management, decision making and meal preparation
  • Assistance with learning, improving and maintaining skills

RESPITE CARE

Respite is a temporary living arrangement (up to 30 days per calendar year) for people living in In-Home supports or Host Home situations to provide caregiver relief. These services can be also be provided in the Natural / Host Home or in a DDA approved Respite Home. This service is available on an hourly or daily rate and can be utilized as stated above up to 30 days per calendar or 720 hours per year.

DAY SUPPORTS

At Accredited Care Services, we provide 100% community-based, customized day habilitation services that promote community inclusion and personal growth. Based on the philosophy that lifelong learning and enrichment opportunities lead to a more meaningful existence, our program is driven by the needs and desires of the people we serve. We believe that every working-age person, disability or not, can work in the community for a fair wage. For working age people we foster and support employment skill building through volunteerism and experiential learning opportunities.

Accredited Care Services launched a new and innovative service, which offers those we serve the ability to ‘create their own day’ and promotes ‘person-centered thinking’. Person-centered thinking is a philosophy behind service provision that supports positive control and self-direction of people’s own lives.

Accredited Care Services Individualized Day Supports provides a person served a ‘Community Navigator’ and ‘team mate’ based on matching goals and interests. Community Navigators guide the group throughout the community and act as facilitators and planners to help those we serve accomplish their goals. Community Navigators are trained advocates and work to promote independence with those they serve. All Individualized Day teams are based on a 1:2 ratio.

During intake, those we serve and their circle of support participate in a discovery process which helps us unearth which work readiness, enrichment and community involvement activities best fit their interests and goals.

Conferring with the support system, the person served develops short and long-term SMART goals with the assistance of ACCREDITED CARE SERVICES staff. Smart goals are specific, measurable, attainable, realistic and timely. Progress towards goals is tracked daily. At the end of each day persons served meet with their Community Navigator and teammate to discuss progress towards goals, and praise and polish points.

The main priority of our day habilitation program is foster community integration for people with disabilities. We want people to have the same opportunities as anyone else in their neighborhood and work and live beside their neighbors with natural supports. Currently, we partner with local organizations, such as Food and Friends, Martha’s Table, Bread for the City and several others to provide those we serve volunteer opportunities.

Community integration activities include:

Supported employment has traditionally been provided for people for whom competitive employment has been difficult to attain, maintain, or for whom competitive employment has been interrupted or intermittent due to the need for additional or ongoing support. People can receive short-term or long-term support through supported employment. We believe in the power of an inclusive workforce. Our experienced team of disability employment professionals work closely with clients to ensure success throughout the employment process providing supported employment services, including assessment, job development, job placement, job coaching and other supports necessary to achieve and maintain employment. Accredited Care Services offers a comprehensive program to help you find your next opportunity or maintain your current employment! Some of the services we offer through supported employment include:

 

  • Creation of Individual Employment Plan: Accredited Care Services Employment Specialist are trained in providing Customized Employment services and can provide job seekers connections to many local District of Columbia employers.
  • Interview Preparation : Are you prepared when an employer calls you for a phone interview? Do you have questions prepared to ask the employer? Accredited Care Services Employment Specialist happy to set up mock interviews and help you organize all your responses for a successful interview!
  • Resume Development: Is your resume highlighting all of your attributes? Allow our Employment Specialist to give you feedback and help you construct the resume for the job you want!
  • Job Fairs and Networking: Our Employment Specialist are knowledgeable and well connected with a variety of sectors and industries. We work hard to establish beneficial relationships in the local business community to accelerate your employment search!
  • Dressing for Success: Need clothing to go to local job fairs or to attend an interview? We have community based resources to help you dress for success!
  • Resources and Training: Accredited Care Services knows you may need additional education, training, or certifications once you begin a job. We can help you get connected with local resources to further excel in your career!
  • Employment Coaching: Job coaches are individuals who specialize in assisting individuals with disabilities to learn and accurately carry out job duties. Job coaches provide one-on-one training tailored to the needs of the employee.
  • Transportation/Moving around the city: Travel Training is key to get to and from work. Accredited Care Services Travel Training for program participants to learn to use the fixed-route public transit services (bus and train) to get to work. One of our Employment Specialist will work with you to design individualized training to use public transportation to get to a location of your choosing
Vocational Rehabilitation Services:

The Vocational Rehabilitation (VR) program provides vocational and rehabilitative services to individuals with disabilities to help them prepare for, secure, regain or retain employment. Persons with disabilities face some challenges in today’s modern workplace. We believe that people with disabilities, given the right opportunities can work and be fully integrated into mainstream society and the work place.
